What is a Medical Home?
Our practice has been designated a Level III Patient-Centered Medical Home since 2010.
The National Committee for Quality Assurance defines Patient Centered Medical Home as a health care setting that facilitates partnerships
between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by information technology and health information exchange to assure that patients get care when and where they need and want it.
